Deadline to submit the form is the first Friday of Fall/Spring semester. 3. Lockers will be assigned according to instrument size. 4. In order to avoid confusion, DO NOT SUBMIT MORE THAN 1 FORM. - You must be enrolled as a student at Florida International University to use a SOM locker. - You will provide your own lock for the assigned locker. - Florida International University School of Music accepts no responsibility for damage to, theft, or loss of the contents of lockers. Be certain that your instrument and other personal equipment are adequately insured against damage, loss, or theft. - DO NOT LEAVE YOUR INSTRUMENT OVERNIGHT IN ANY AREA THAT IS NOT YOUR LOCKER. - You understand that the locker rental will end at the end of the academic year and that the University may dispose of any contents left in the locker after designated deadline removal date. - You understand assigned lockers may not be changed without notifying the School of Music main office. - Abuse of lockers may result in the loss of locker rental privileges.
