Academic Amnesty/Academic Salvage Request Form

The University has two policies which will recalculate a student’s GPA that previously showed poor performance at FIU.

This form must be completed by the student prior to the Academic Department’s review of the readmission file under said policies. All former undergraduate students requesting Salvage/Amnesty must first reapply for admission via the online application as well as submit this form.
