FIU Health Compliance Insurance Checklist

This form allows you to upload proof of your alternative health insurance to the FIU Health Compliance Department. Since you have chosen to purchase a health insurance policy that is not the FIU-sponsored plan or one of the pre-approved health insurance plans, please note that the Health Compliance Office cannot review any alternative health insurance plan without the completion of the FIU Health Compliance Insurance Checklist.

Why You Are Completing This Form

Your insurance is through an Employer, Private, Family, or Other Plan that is NOT Pre-Approved by FIU.

Since you have chosen to purchase an alternative insurance plan, you and your insurance company must complete and submit the following required documents:

1. International Student Alternative Health Insurance Compliance Form
2. FIU Health Compliance Insurance Checklist
3. Certificate of Coverage (Policy Details)
4. Insurance Card (Front & Back)
5. Proof of Pre-Payment

All required documentation must be submitted through the FIU Health Compliance Insurance Checklist. If the checklist and all required documents are not submitted, your insurance cannot be reviewed, and this may cause delays in the registration process.

If your documentation is complete and accepted, your health insurance hold will be lifted, and you will be able to register for classes.

If your submission is incomplete, it may delay your registration.

After the 48–72 hour review period, you may log in to my.fiu.edu to check if your insurance hold has been removed.
