Co-Curricular and Experiential Learning Transcript (CCELT) Industry Credential Verification Form

The purpose of this form is to allow a student to self-report an earned industry credential that can be verified and posted on their Co-Curricular and Experiential Learning Transcript.

You are completing this form because you have earned an industry credential while you were an active student at Florida International University (FIU) and are requesting to have it featured on your Co-Curricular and Experiential Learning Transcript (CCELT). To move forward, you will be required to upload proof of your credential along with a link that confirms it was awarded to you.

Eligibility: Your credential qualifies for the CCELT only if you earned it as an FIU student. We accept self-reported digital credentials and professional certificates (such as those issued by HubSpot Academy, Google Skillshop, Amazon Web Services, etc.) that can be verified by an official source. All supporting documentation submitted through this form will be reviewed and verified by the Office of Credentialing Learning.

Minimum Requirements:

Recognized Issuing Organization: Your credential must come from an established and reputable organization within its industry. This reassures everyone that your achievement meets professional standards.

Verifiable Credential Link: You must provide an official digital link—this can be a digital badge URL or a Credential ID/URL—that shows your name, the credential earned, the issuing organization, the issue date (and expiration, if any), and the relevant skills or standards.
