I understand the following policies and procedures regarding this application:
1. IN ORDER TO BEGIN THE DIETETIC INTERNSHIP STUDENTS MUST:
a. Have completed ALL their didactic course requirements and have a DPD verification statement.
b. Have completed ALL the MS course requirements.
c. Complete the MS thesis/project requirement, including presentation in Graduate Seminar (DIE 6937) if applicable.
d. Doctoral students should have approval signature of major professor. Effective fall 2020, students admitted to the doctoral program fall 2020 or after will be required to complete the internship either prior to admissions into the doctoral program or after official graduation from the doctoral program.
2. The student will be responsible for his/her own transportation to the supervised practice site. Internship sites are located across four counties and your placement may require you to travel to one of these counties. Students assume all liability for safely traveling to and from assigned areas.
3. The DI is designed for FIU graduates from the Department of Dietetics and Nutrition.
4. The DI applicant must have a COMPLETED Level II FBI Background Check including the Finger-Printing completed through American Data Bank. The student is responsible for any costs that this requirement incurs. The DI applicant understands that final admission to the DI is contingent on the outcome of their background check.
5. There is an additional fee of $50 per credit hour assessed for internship classes, for the purpose of meeting the expenses associated with the internship education.
6. Students must abide by the Academy of Nutrition and Dietetics’ Code of Ethics for the Profession of Dietetics. Students must also abide by the standards as discussed in the FIU DI Handbook. Students will not be admitted into the DI if their conduct does not meet these codes. During the Dietetic Internship, any student who does not perform in accordance to applicable policies and procedures as outlined in the DI Handbook, Academy Code of Ethics and FIU Student Handbook, may be removed from the Dietetic Internship Program.
If accepted into the DI, before supervised practice begins, each student will need to provide documentation:
a. Vaccinations, including varicella titer, PPD X 2, MMR, meningitis, hepatitis B and tetanus vaccines.
b. Active adult CPR certification (only by American Heart Association).
c. A 10-panel drug test

If I fail to comply with the requirements of the Dietetic Internship, I forfeit my acceptance into the internship. I certify that the information contained in this application is correct to the best of my knowledge. I understand that if I have falsified information, not met eligibility for application requirements and/or misrepresented myself, my application will be withdrawn.
