This forms serves as documentation to justify exception of using one of the Preferred Caterers due to extenuating circumstances. Upon approval, this form should be submitted as part of your supporting documentation for payment reconciliation to FIU Purchasing/Procurement.

PREFERRED CATERING GUIDELINES

All Catering Services that are expensed using University funds are required to utilize the Preferred Caterers, unless otherwise exempted in writing by the Office of Business Services for extenuating circumstances.

“FIU Foundation Funds” means a specific source of academic funding used by the Ordering Department, and the corresponding payment process by the University, to pay invoices submitted to the University. Catering Services paid using FIU Foundation Funds are exempt from the requirement of utilizing the Contract, and the Ordering Departments are not required to choose from the Approved List for catering events.

NOTE: Last minute need for catering due to lack of planning does not justify as an extenuating circumstance.
