FIUMB Color Guard Equipment Loan Form

I accept full responsibility for the equipment described and loaned to me by Florida International
University School of Music, and I will return it, together with all accessories, by January 31.

• If it is damaged, lost or stolen, before I return it to the Music Department, I will take full responsibility for repair or replacement costs.

• Rental fee of $35 per semester. An additional late return fee of $35 will be charged if returned after January 31.

• I agree that Florida International University School of Music may, at any time and under any circumstances appropriate the equipment specified in this agreement.

• I agree that Florida International University may impose a hold on my records and financial aid under these circumstances:
a. That I do not return the specified equipment as agreed
b. That I have not paid the rental fee or any costs of replacement or repair for which I am responsible.
