I accept full responsibility for the instrument described and loaned to me by Florida International University School of Music, and I will return it, together with all accessories, by January 31 from Fall Semester or by March 31 for Spring Semester. If it is damaged, lost or stolen, before I return it to the Music Department, I will take full responsibility for repair or replacement costs. Replacement costs range from $1-6,000.00 depending on instrument you are renting out. Rental fee of $35 per semester. An additional late return fee of $35 will be charged if returned after January 31 for Fall and March 31 for Spring Semester. I agree that Florida International University School of Music may, at any time and under any circumstances remove the instrument from any music locker or storage facility without prior notice. The department shall not be responsible for instruments stored in music lockers whether shared or individually occupied. I agree to return my instrument directly to Shelly in the W06 office. (not to a section leader or other band member on my behalf) I agree that Florida International University may impose a hold on my records and financial aid under these circumstances: a. I did not return the specified instrument as agreed b. I have not paid the rental fee or any costs of replacement or repair for which I am responsible.
