Radiation Dosimeter Assignment and Cancellation Form

All individuals working with radioactive materials are required to wear a dosimeter to monitor exposure. Dosimeters must be assigned PRIOR to starting work and must be canceled once the work has ended and/or PRIOR to leaving the university.
